Once you have a spot please fill out your registration form and portable record form. The portable record form has the same information as the registration form, the key difference is that it gives us permission to seek medical treatment in the event of an emergency. We bring it on all outings as well as have a copy in the office at all times. Both of these forms are mandatory before starting the program according to government legislation.
